Our Mentorship (HCBS–SLS) service at HOG Home Care LLC is dedicated to helping individuals with developmental and intellectual disabilities achieve greater independence, confidence, and self-determination. Through personalized guidance and consistent support, we empower participants to develop essential life skills, build meaningful relationships, and reach their personal and professional goals.

• Individualized Mentorship Support: Our mentors work one-on-one with each participant, offering personalized guidance that aligns with their unique strengths, needs, and aspirations.
• Skill Building and Growth: We focus on enhancing daily living, communication, decision-making, and social interaction skills—empowering participants to live more independently and confidently.
• Community Integration: Mentors encourage active participation in community life by connecting individuals to resources, social opportunities, and programs that promote belonging and inclusion.
• Goal-Focused Development: Every mentorship relationship is guided by the individual’s Person-Centered Plan, ensuring that support activities directly contribute to their personal growth and self-sufficiency.
