Community Connector (HCBS–CES)

Our Community Connector (HCBS–CES) is designed to empower individuals with developmental and intellectual disabilities to live fuller, more independent, and socially connected lives.

• Individualized Community Engagement: We help participants explore community resources such as recreational centers, libraries, volunteer opportunities, classes, and social events that align with their goals and interests.
• Skill Development: Support in learning and practicing social, communication, and life skills that promote confidence and self-sufficiency.
• Building Natural Supports: Encouraging connections with peers, neighbors, and community members to create lasting relationships beyond formal care.
• Goal-Oriented Activities: Every outing or activity is designed to align with the individual’s Person-Centered Plan, supporting personal development and independence.
• Safety and Support: Our trained staff ensure that every activity is carried out in a safe, inclusive, and supportive manner—empowering participants to take part at their own comfort level.
